Health and Welfare Lasting Power of Attorney
A Health and Welfare LPA enables you to choose the person you would like to make personal decisions on your behalf, this includes;

- Day to day care
- Whether you should take part in social activities, leisure activities, education or training
- Who you may have contact with
- Consenting to or refusing medical examination and treatment on your behalf
- Rights of access to your personal information
- Assessments for and provisions of community care services
- Your personal correspondence and papers
- Complaints about your care or treatment
Your attorneys can make decisions about your personal welfare, such as giving or refusing consent to particular types of health care, including medical decisions. You can include matters such as whether you would want to stay in your home with appropriate care or, if you did have to go into a care home, the issues that are important to you in the choice of a home.
